• The film's working title was 'Dheera: The Brave', but it was shortened to just 'Dheera' before release.
• Lead actor Vikranth Srinivas, who also directed, performed many of his own driving stunts after intensive training.
• The entire road trip from Vizag to Hyderabad was shot on real locations, not in a studio, to capture authentic landscapes.
• A real ambulance was modified for key action sequences, including a challenging scene where it gets a flat tire.
• The film's climax was shot during a rare, heavy rain spell in Hyderabad, which was not originally planned.
• Composer Mani Sharma recorded a portion of the background score with a live orchestra in a Hyderabad studio.
• The character of the patient was played by a newcomer who had to maintain a completely still, unconscious posture for hours during filming.
